You keep a watch on the IP of the client who did the last frag - most likely he was either lagging terribly at that point (so the anticheat got confused, it's still not perfect) or he was using a modified client which the anticheat noticed. The bailout isn't the proper response though, Brahma should've just left a log-message instead.
But actually - I notice your server is the same one that has come up in another thread with 3 demos being unreadable .. so maybe your server has some issue itself.
