Don't put modified data into the installation-home - only full-fledged MODs (stuff you run by passing "--mod=" to the binary goes into the mods/-directory .. anything else should be put into your game-home (what you pass with "--home=" to the binary).
That way you can always revert to a vanilla-game without any trouble.
Such files aren't ever overwritten by the engine itself, so the fault must lie on your hardware - or as was suggested some software (malware) or wetware (siblings) we have no control over.
