• High-velocity rounds can indeed do very nasty things to all body parts.
  
• Grenades do NOT cut you into handy pieces of meat - they just pepper you with shrapnel.
• IRL, no one would ever slice with a combat knife - those things are made to A) cut through razor wire or brush and B) to stab.
  
• Shotguns do NOT blow the victim away. That's all Hollywood nonsense. According to physics, if shotguns were powerful enough to do that, the shooter would be propelled backwards as well.
  

Generally, I don't think this has to be discussed. The effects modern weapons can cause are gruesome enough in reality, you don't have to depict that too exactly in games.
