I see these every so often and I was just wondering what the significance was.
It's appears as "doomer (10)".

When two people on a server have the same name their client number on the server is put next to them in parenthesis and orange.

That's what is happening. The significance here is that based on the client number you can tell which one is which. For instance if someone changes their name to someone else's on the server and begin teamkilling, you can tell which one is which by looking at the client number next to the name and score. This way a kick-vote can be done correctly.
